Baffle Repair for Transposition of the Great Arteries
نویسندگان
چکیده
Forty-nine patients with transposition of the great arteries who underwent a Mustard atrial baffle repair between 1964 and 1971 were assessed late postoperatively. There have been five late deaths: two related to baffle obstruction, two from noncardiac causes, and one sudden and unexpected. Hemodynamic data were available in 42 patients and autopsy in four. Obstruction of the lower venous channel was not encountered. Three patients had severe obstruction of the upper venous channel and in four there was mild restriction. Two patients had severe pulmonary venous obstruction resulting in late death; mild asymptomatic obstruction could not be excluded with certainty in six patients. Tricuspid incompetence was

Factors influencing choice of procedure in transposition of the great arteries: a decision analysis approach.
Clinicians have a difficult choice between the arterial switch (Jatene et al.) and the atrial baffle operation (Mustard or Senning) for transposition of the great arteries. The surgical decision is essentially a determination of whether the long-term course after the arterial switch procedure (thought to be more favorable than after the atrial baffle procedure) plus elimination of presurgical a...
متن کاملRepair of Transposition of the Great Arteries
From May 1963 to July 1972, 123 patients ranging in age from 20 days to 15 years, with Dtransposition of the great arteries, underwent an intraatrial baffle operation using a pericardial baffle. They previously had had a total of 121 various palliative procedures. Twenty-six children (21%) died in the early postoperative period. The operative mortality was 13% in transposition of the great arte...
متن کاملHemodynamic data and angiographic findings after Mustard repair for complete transposition of the great arteries.
SUMMARY Eleven childrern with uncomplicated transposition of the great arteries repaired by the Mustard procedure were studied by atrial pacing, cardiac catheterization, and ciiieangiography. Nine had persistent postoperative ectopic cardiac rhythms. Atrial pacing in five suggested sinus node damage in four and A-V junctional conduction tissue damage in one. In seven children dye-dilution curve...
متن کاملEchocardiographic visualisation of the interatrial baffle after Mustard's operation.
Thirty children aged from 7 weeks to 14 years were examined by echocardiography after Mustard's operation for transposition of the great arteries. Discrete and persitent echoes were noted within the original left atrial cavity and contrast echocardiography was used to establish that these originated from the interatrial baffle. In the presence of caval channel obstruction, caused by malposition...
متن کاملPulsed Doppler echocardiographic detection of obstruction of systemic venous return after repair of transposition of the great arteries.
Pulsed Doppler echocardiography (PDE) allows the detection of direction and quality (smooth vs disturbed, or "rough") of blood flow characteristics at known sites within the heart and great vessels. In order to test the ability of this technique to detect obstruction of systemic venous return after interatrial repair of transposition of the great arteries, twelve children who had undergone such...
